Outbound Assignments

If you have been notified of an Assignment and have not received an e-mail from your Assignment Counselor after 7 duty days, please email Career Development at 75FSS.FSPD.CareerDevelopment@us.af.mil or call (801) 777-1673 Opt 2.

If you need to schedule a final out appointment, please contact us and ensure your vOP checklist is 100% complete.


Retirements/Separations

If you have any questions or concerns related to Retirements/Separations or have an approved Retirement/Separation, please email Career Development at 75FSS.FSPD.CareerDevelopment@us.af.mil or call (801) 777-1673 Opt 2.

Reenlistment/Extensions

If you are in your window to reenlist, Reenlistments require an Article 137 brief training via Air Force myLearning and a signed Article 137 Briefing Memo conducted by the legal office within 30 days of the projected reenlistment.

In order to extend, please ensure that you meet an extension reason which is listed in AFI36-2606, Table 6.2.

Promotions

Evaluations

If you have a missing EPR/OPR, please contact us at 75FSS.FSPM.ForceMgmt@us.af.mil or (801) 777-1673 Opt.4

All other questions/concerns, please reach out to your CSS.

Any EPR/OPR that is wanting to be changed/removed from your records will have to go through the ERAB (Evaluation reports appeal board) through vMPF.


Awards & Decorations

If you have a missing/erroneous Award and/or Decoration, please contact your CSS so they may update it.

Inbound PCS Amendments

If you are PCS’ing/have PCS’d to Hill and encountered any updates/corrections to your Orders (AF 899), contact Force

SDAP & Duty Info Updates

Any changes to SDAP (Start, Change, Stop) or to update/correct your Duty Information (Rater changes, PAFSC, DAFSC, CAFSC, SEI, Duty title), please contact your CSS for update/correction or initiate an AF 2096.

Installation Personnel Readiness

For any missing/incorrect deployment information, deployment-related concerns (to include out-processing for a deployment or in-processing upon return from your deployment), contact your UDM or 75FSS.FSOXI.IndPersR@us.af.mil or call (801) 777-3498.


Official Passports

ONLY personnel due to PCS or go TDY Overseas that need official passports can contact the Passport Agent.

Personal/Tourist passports are NOT processed through this office. Please visit your local USPS branch.
